Reimbursement Procedure:

Procedure for getting reimbursed for food after the fact:

1) Collect original itemized receipts/ Make copies for yourself
2) Fill out "Request for Reimbursement" form
3) Fill out the "Entertainment and Student Programming" form as much as you can.
3) Sign in the "Certification by Host" box
4) Send forms and original receipts to Kara Willett at the CCWP office:

Here is the procedure for getting reimbursed for non-food items:

3) Attach receipts to reimbursement form and give to Kara Willett at the CCWP office:

Here is the procedure for getting reimbursed for travel:

1) Collect original itemized receipts (including a copy of your credit card statement with only the last four digits of your credit card visible)/ Make copies for yourself
2) Fill out "Request for Reimbursement" form
3) Fill out "Post Travel" form as much as you can.
4) Make sure to sign this document
5) Give forms and original receipts to Kara Willett at the CCWP office:

1) Figure out your estimated budget
2) Fill out the "Request for Reimbursement" form
3) Fill out the "Travel Advance" form as much as you can
4) Make sure to sign this form.
5) Give this form to Kara Willett at the CCWP office:
